Creating and Managing Forms

This guide explains how to create and edit a customer Form, choose the registrations and memberships it applies to, set its Timing, add Fields, and share a direct link to it, so you collect the right information at the right point in a customer's journey.

📝 Overview

Forms let you collect information from your Customers using Customer Fields, such as emergency contacts, health questions, or how they heard about you.

This guide walks through creating a Form, deciding which registrations and memberships it applies to, choosing when it must be completed, adding Fields, sharing a direct link to it, and editing it later.

A Form is a set of Fields that Customers complete during signup, registration, or purchase, so you gather the right details at the right moment.

Before building a new Form, it is worth reviewing the pre-loaded Forms that already exist in your account, as the data you need might already be collected. In that case you can simply edit an existing Form instead.

🛠️ Setup

You create a new Form from your Forms area, then set its name, requirements, Timing, resubmission preference, and Fields.

To start, go to Forms/DocumentsCustomer Forms ➜ click Add a Form.

Once you have clicked Add a Form, complete the following steps.

1️⃣ Name your Form

Enter a name for your Form. The wording you use is shown to Customers in the registration and purchase flow, so keep it clear.


2️⃣ Choose what the Form is required for

Decide which registrations to events (classes, courses, and appointments) and which membership purchases should prompt the Form. Within each area you can choose one of the following options.

All registrations/memberships

Every Customer registering for any event, or buying any membership, is prompted to complete the Form.

No registrations/memberships

Customers are not prompted to complete the Form, regardless of what they register for or buy. This is useful if you are not ready to collect the Form yet, or want to pause an existing one.

Only selected ones

Choose the specific registration types or membership purchases that should prompt the Form, for example only a monthly membership or only your beginner classes.

All except those selected

The Form defaults to being required for everything except the specific types you select, including types you create later.

⚠️ Registration options are based on your Class Types. To require a Form for one specific session only, such as your 6am classes, set that session up as its own Class Type first.


3️⃣ Set the Timing 🆕

Timing controls when a Customer must complete the Form in relation to their registration or purchase. This is a newly released option in the Forms flow.

🔔 New: Forms can be required Before a registration or purchase, After it, or on a Customized basis, choosing Before or After individually for each class, course, appointment type, registration, or membership. This is helpful for free trials where you want signup to stay quick.

4️⃣ Choose whether the Form should be resubmitted

Decide whether Customers should be asked to complete the Form again after a set period, for example to keep health-related answers or emergency contacts up to date. Make your choice, then click Next.

5️⃣ Add your Fields

On the final page, choose which Fields to include by clicking Add next to each one. Once you have added your Fields, the Form is ready to use.


If you need a Field that is not yet in your Customer Field list, click Create a Field to add it while building the Form.

If you have a long list of Customer Fields, use the Search function to find the ones you need.


🔁 Sharing a link to your Form 🆕

🔔🆕 Each Form now has its own unique link. You can copy it and share it however you like, or email it to a Customer directly from your dashboard, so they can complete the Form before reaching the usual trigger point.

Alongside prompting Customers at the registration or purchase point, you can share a direct link to a Form so a Customer can complete it in advance.

You can still direct a Customer to the Forms and Waivers tab on your Customer Site, or simply prompt them to go through the trigger point such as registering for an applicable class.

✏️ Editing a Form

You can edit an existing Form at any time from your Forms area, changing its details, requirements, Timing, and Fields, and previewing the result.

🔔🆕 The editing flow has been updated. Editing a Form now takes you through the same steps as creating one, so you can adjust any part of it in one place.

To edit a Form, go to Forms/DocumentsCustomer Forms ➜ click Options next to the Form in question ➜ Edit. From there you can work through the following steps.

Form Details

Edit the Form Name and choose whether Customers are asked to resubmit the Form after some time.

Requirements

Change which registrations and membership purchases prompt the Form.

Requirement Timing

Change whether the Form is required Before, After, or on a Customized basis for each selection.

Add/Remove Fields

Add or remove the Fields included on the Form.

Preview

Review how the Form looks to Customers before saving your changes.


✏️ Editing the Signup Forms

The Signup Form and the Non-Participating Signup Form work differently from your other Forms. They cannot be edited through the steps above, and you can only add or remove their Fields.

To manage them, go to Forms/DocumentsCustomer Forms ➜ click Edit Details to the right of each. From the Form Fields page you can add or delete Fields, create a new Field, search your Fields, and preview the Form.


🧑‍💻 Customer View

Once your Form is set up, it appears in the registration or membership purchase flow on the Customer Site, based on the requirements and Timing you selected.

🧠 Example:

A Form set to Before blocks the registration or purchase until the Customer completes it, while a Form set to After lets the Customer finish booking first and complete the Form afterwards.

Can I require a Form before or after a registration or purchase?

Can I share a direct link to a Form?

While adding Fields, click Create a Field to build a new one. It is added to both the Form and your Customer Field list in a single step.

Can I edit a Form after creating it?

Yes. Go to Forms/Documents ➜ Customer Forms ➜ Options next to the Form ➜ Edit to change its details, Requirements, Timing, and Fields. The Signup Form and Non-Participating Signup Form only support adding or removing Fields via Edit Details.
