You can target specific customers to complete certain forms based on the registrations or purchases they make. These forms are submitted by customers the first time they make an applicable registration or purchase on the Customer Site.
For example, for a Pre-natal Yoga class you might collect relevant details such as a due date or any pregnancy-related issues. Those questions only apply to that class, so you would make the form applicable to that registration only.
For an Olympic Lifting class you might collect more general information instead. A separate form, the "New Joiner Form" in the example below, could apply to that registration and to all other registrations except Pre-natal Yoga. You could also ask your one-to-one clients to complete a fitness assessment before their first appointment.
You can also add a membership requirement to these forms. In the example below, the forms are required only for class registrations, not for purchasing any memberships.

š ļø How to create the form
These forms are managed from Forms/Documents ā Customer Forms. To add a new one, click Add a form.
On the next page, add the form name, set what it is required for, and choose whether customers should resubmit it periodically to keep the data fresh. Click Next to select the fields that should be asked in the form.
š§āš» What customers experience
When a customer books an applicable class, they are asked to complete the relevant form before they can register.
Here is what they see when booking from the Customer Site:
If they choose the Pre-natal Yoga class, they complete the pre-natal form in order to register:
If they choose the Olympic Lifting class, they complete the new joiner form before registering:

With some exceptions, a submission lasts for life. If a customer has already submitted the form, they will not be asked to complete it again.

Can I share a form link with a customer?

Yes. Each form has its own unique link that you can copy and send to a customer, so they can complete it directly. Customers will also still be prompted to complete an applicable form when they go through the relevant trigger point, such as registering for an applicable class or purchasing an applicable membership.

Can I create a form specifically for store purchases?

No. Forms can be required for class and course registrations and for membership purchases, but it is not possible to create a form specifically for Store purchases.
If you need to collect information from these customers, you could set a form to apply more broadly, or share the form link with them directly.
